I discuss with the client to find out what ideas they have for the product then touch up the ideas so that they are design feasible and make suggestions to make them better. I find out what sort of consumers the client hopes to reach and figure out the mindsets of these potential consumers. I draw up a flow diagram to simulate how the potential user would go through the product. I then sketch out what the product could look like using the user personas and user flow diagrams as guidelines. I create wireframes and present them to the client to make changes and generally give the go-ahead. I create visual mockups and prototypes with the look and feel of the product, present to the client and make changes based on the client's wants while suggesting best practices to the client. I do some A/B testing with a small audience to see how the potential users feel about it and implement changes based on this. The final step is to launch the product and receive feedback from actual users and make changes accordingly.
